Almost every competition I've looked at requires that your work has not been published.
What exactly does that mean these days? I've been in a local newspaper (yes?), on websites (no?), on a BBC website and TV station (hmm..) - your help is most welcome to put me right!

it usually means all of the above - competition entries these days are often required to be unique as subsequent licensing is also part of the competition structure.
rules is rules. thats what they are there for. there would be little point entering a competition knowing you are breaking the rules and subsequently risking disqualification.
it would depend on the rules specific to the competition - they are normally quite explicit in their description. Personally, I would doubt that minor tweaking in an attempt to circumvent the rules would be tolerated anymore than posting the already published material. Most photographic contests do not permit manipulations other than basic enhancements such as sharpening anyways.
